SaaS provider Brightree announced plans to connect its tens of thousands of home health and hospice customers with 50,000+ provider locations and health systems around the country via the CommonWell Health Alliance, and through CommonWell to Carequality. As the first CommonWell integration focused solely on post-acute care, this is a major milestone in improving interoperability for post-acute care providers and their patients, solving for the “black-hole” phenomenon that occurs when patients become lost to follow-up upon their entrance into post-acute care.
Traditionally, home health and hospice agencies obtained critical patient health data manually from each of a patient’s other care providers. This required significant resources and often resulted in large gaps in patients’ records, hindering the speed and quality of care patients received.
With this connection, Brightree’s home health and hospice customers will be able to easily and quickly retrieve documents and data from their patients’ previous hospital and/or physician visits within their EMR solution and can share their updates with the physicians and providers in the CommonWell/Carequality networks who serve that patient.
