The Connecticut Hospital Association tapped Unite Us, the New York City-based developer of a social care coordination platform, to help build a digital statewide network of health and social services providers.
The platform will connect the state’s healthcare providers with local social services agencies. Hospitals and health systems will then be better equipped to direct patients toward organizations that can aid them in finding housing, food, transportation, employment and more.
“With the Unite Us technology platform, Connecticut’s hospitals and health systems, in collaboration with community-based organizations, will be better equipped to address patients’ needs from a holistic standpoint. This is one example of how Connecticut hospitals and health systems are transforming healthcare and keeping our communities healthier,” Jennifer Jackson, CEO of the Connecticut Hospital Association, said in a statement.
