Vively Health and Cerner agreed to a five-year partnership to help patients with chronic conditions.
Vively Health, a home healthcare provider for vulernable patient populations, announced a partnership with Cerner EHR to improve care delivery, improve health outcomes for patients with multiple chronic conditions, and push its population health mission.
Cerner’s Millennium EHR, its population health management platform, HealtheIntent, and its consumer framework will be implemented across Vively Health. Cerner’s consumer framework is designed to help lower the cost of care by supporting care teams to better address the unique needs of what Vively calls its Most Vulnerable Patients, or MVPs.

Vively will utilize Cerner’s integrated platforms and technology to support an improved patient experience through seamless care coordination, the company said. Cerner’s technology has helped reduce the intricacies of healthcare and drive meaningful advancements, directly benefiting Vively’s consumers.
Using Cerner’s Millennium EHR, Vively can now provide a more comprehensive picture of an individual’s health and financial information on a single platform, helping both the clinician and the patient. Having all information on one platform increases efficiencies within patients’ homes, which enhances the ability of the clinicians to surface issues prior to and during care visits.
Vively says Cerner’s HealtheIntent population health platform will help its clinicians predict the health of their patients. Specifically, the home health providers hopes the tool will allow providers to better understand and track its patients’ health by aggregating and normalizing multiple data sources into a single, consolidated view. Providers will also be able to share this health data with other clinicians, hospitals and care organizations, to help the patient at all times.

According to the Centers for Disease Control and Prevention, 60 percent of adults in the country live with a chronic condition, and 40 percent of adults live with multiple chronic conditions. In fact, 12 percent of individuals with five or more chronic conditions drive 41 percent of US healthcare spending.
Vively primarily delivers care to individuals with diabetes, cardiovascular disease, chronic kidney disease, COPD, depression and anxiety.
Cerner is making a number of healthcare partnerships as of late, with many major industry players announcing that they will use aspects of the EHR vendor’s platform.
Earlier this month, Geisinger announced a new ten-year agreement with Cerner to implement its HealtheIntent platform for improved population health management.
Geisinger will use actionable insights and innovative tools from the Cerner platform across its patient network, accountable care organization (ACO), and Geisinger Health Plan. The partnership will use open standards and integration capabilities to provide a seamless user experience.
Geisinger will utilize Cerner’s analytic capabilities to extract actionable insights from patient data. The partnership will help manage patients’ health and further support the transition to value-based care.
