Enclara Pharmacia, the market leader in comprehensive pharmacy benefits management (PBM) services to the hospice community, is pleased to announce its completed acquisition of Avanti Health Care Services, a full-service, direct delivery pharmacy provider in the greater New York region. The acquisition represents a strategic expansion to optimize medication access options and to support underserved regional markets, as well as Enclara’s expansion into the home care pharmacy market.

The acquisition expands Enclara’s clinical expertise and resources to already successful medication therapy management (MTM) programs at Avanti in the home care market. With its focus on chronic care populations, the company’s MTM program offers comprehensive reviews by specially trained pharmacists that can lead to better clinical and economic outcomes, including fewer hospitalizations, improved medication adherence and better management of chronic conditions from within the patient’s home. It also expands Enclara’s service offering for clients beyond solely hospice-covered medications.
