Summit Overview
The Collaborative Care & Health IT Innovation Summit, formerly known as Long-Term and Post-Acute Care (LTPAC) Health IT Summit, will be held in Baltimore, Maryland, June 23-25, 2019.
This event is the premiere health IT conference for executives and information technology leaders from LTPAC sector, as well as acute care providers, payers, and technology vendors serving the older adult and individuals with chronic conditions throughout the continuum of healthcare. The summit is convened by LeadingAge CAST and will continue to focus on bringing leaders together to discuss how technology is connecting the healthcare ecosystem in the era of health and payment reforms.
