HealthlinkNY has secured connection funding and is currently offering New York-based healthcare organizations up to $89,000 to engage in health information exchange use until September 30, 2018.
The HIE will offer discounts up to $76,000 to healthcare organizations that connect to the HIE. Additionally, the New York eHealth Collaborative is offering healthcare organizations $13,000 to connect and engage in health data exchange with their local HIE.
Healthcare organizations will receive funding on a first-come, first-serve basis.
Organizations can receive a full HIE connection including admission, discharge, and transfer notifications and alerts and other HIE services for a total of $10,000.

NYeC offers healthcare organizations three incentives including the Data Exchange Incentive Program, SHIN-NY Connections Initiative Program, and the New York State Patient-Centered Medical Home.
As part of DEIP, eligible practices can receive up to $13,000 in incentives to connect with an HIE and contribute patient health data to the exchange.
Organizations with at least one provider that has successfully attested to the meaningful use program are eligible for the incentive program, according to HealthlinkNY. Additionally, long-term care and behavioral healthcare providers can receive incentives.
Providers can also receive up to $13,000 for participating in the SCI Program.
“The SHIN-NY Initiative Program is a part of New York’s ongoing work to transform healthcare delivery and shift towards value-based care,” stated HealthlinkNY.
“Focused on small and medium practices who are struggling to navigate the changing landscape, New York eHealth Collaborative’s SHIN-NY Initiative Program practice transformation services help practices deliver high-quality, coordinated care, earn payment incentives, and prepare to thrive under value-based payment arrangements,” clarified the HIE.
Small and medium-sized practices that participate in the SHIN-NY Initiative Program will function like Patient-Centered Medical Homes. PCMs are designed to boost patient health outcomes at both the individual and population level while reducing healthcare organization costs and improving the patient experience.
PCMHs achieve this aim by prioritizing enabling care coordination through EHR technology, health data exchange, data analytics, and population health management tools.
Practices enrolled in the SHIN-NY Initiative Program will receive up to two years of free technical assistance and support from NYeC to prepare for value-based payment arrangements.
“NYeC will support practices in Western New York, Westchester County, Rockland County, New York City, and Long Island,” stated HealthlinkNY.
Finally, healthcare organizations can receive incentive payments through the NYS PCMH program. The program is a collaborative between the New York State Department of Health and the National Committee for Quality Assurance to develop a PCMH model exclusive to New York primary care practices.
“Soon, the NYS PCMH program will be the only path for a New York primary care practice that wants to participate in the PCMH model,” clarified HealthlinkNY. “Various payers in New York State offer incentive payments to providers who meet PCMH criteria.”
NYS PCMH is similarly focused on small and medium-sized practices.
“Practices with sites that provide primary care services including internal medicine, family, and pediatrics practices are eligible to participate in NYS PCMH,” stated HealthlinkNY. “NYeC provides technical assistance to both practices that are new to PCMH recognition and those that have already attained PCMH 2014 recognition.”
These incentive payments can help to motivate healthcare organizations to engage in health data exchange and utilize HIE services for better-informed clinical decision-making and more accurate care delivery.
In addition to HealthlinkNY, New York includes a number of other HIEs such as Healthix, HEALTHeLINK, Rochester RHIO, HealtheConnections, and Hixny.
