Cerner announced that Carolina HealthCare System (CHS) will implement the EHR vendor’s population health management platform HealtheIntent for improved coordinated care.
This platform will enable almost 12 million patient interactions. This care management will take place at all care sites that are part of Carolinas Physician Alliance which is clinically integrated a network of CHS.

Cerner’s HealtheIntent collects data in near real-time from several sources to create a comprehensive view of patients experience. The packages that are included are HealtheeEDW, HealtheRegistries, and HealtheCare.
Ruth said further,
“Our goal is to equip our care teams with a holistic and timely picture of a patient’s interactions throughout CHS, as well as provide system-wide insights to ultimately improve quality of care and reduce costs for our entire patient population,”
Craig Richardville, Carolinas Healthcare System CIO, and Analytics Officer said,
“As the healthcare industry transitions to value-based care, the ability to consume all data whether it originates from our own or our affiliate’s electronic medical records, social determinants of the patient, self –reported data or from any sources becomes invaluable. This allows us to create a 360 view of each patient to offer the insight to our care team to deliver appropriate treatment, prevention and to bend the cost curve to make sure our services are more affordable to the patient and increase the health of each patient and of our communities,”
According to Black book Research report, Cerner and Epic are dominating population health management software market. They also include more clinical and administrative functionalities for stratifying risk and tracking patient movement across the care continuum.
