The University of Chicago Medicine and Humana inked a value-based care deal to better coordinate care for Humana members in the Chicagoland area.
Under the agreement, patients with Humana’s Medicare Advantage, HMO, PPO and private fee-for-service health plans will have access to value-based care services at UChicago’s hospital, outpatient care centers and physician group practices. The agreement emphasizes personal time between patients and health professionals, preventive care and screenings, chronic care, using data analytics to enhance coordination, and reimbursing physicians for their patients’ outcomes.
UChicago Medicine Chief Clinical Transformation Officer Raj Krishnamurthy, MD, expressed her enthusiasm for the new partnership and the mutual commitment to innovate and collaborate on care. “Amid the transition to value-based care, UChicago Medicine will continue to enhance its efforts to coordinate care and focus on each patient’s unique health needs,” she said in a press release.
