Georgia Health Information Network collaborates with Aetna Health to launch Georgia Unity, a pioneering social data platform for whole health integration. Addressing rural community health disparities, the initiative leverages social determinants of health to foster trust and improve outcomes. Despite initial challenges, it achieved significant milestones, including substantial cost savings and thousands of referrals to community health resources in Emanuel County.
Georgia Health Information Network (GHIN) has embarked on an innovative partnership with Aetna Health to launch Georgia Unity, a groundbreaking social data platform aimed at achieving whole health integration. The unveiling of this initiative took place at the prestigious HIMSS24 conference, where GHIN representatives shed light on the intricacies of Georgia Unity, a pivotal health information exchange network developed in collaboration with local community partners. This groundbreaking endeavor stems from the pressing need to comprehensively understand and address the health landscape in Georgia’s rural communities.
Leading the charge are Dr. Denise Hines, the esteemed Executive Director at Georgia Health Information Network, and Shirelle King, a distinguished Program Manager within the organization. Together, they articulated the overarching objective of the Georgia Unity project: harnessing the power of social determinants of health to bridge existing disparities in health equity and establish robust frameworks conducive to superior health outcomes.
Dr. Hines emphasized a paradigm shift in healthcare, asserting that conventional perspectives often overlook a substantial portion of health dynamics. According to her, while traditional healthcare models focus merely on the medical aspect, a staggering 80% of health outcomes are intricately intertwined with social determinants such as access to nourishment, adequate shelter, and suitable clothing. It is precisely these facets that the Georgia Unity initiative seeks to prioritize and address comprehensively.

At the core of Georgia Unity lies the imperative of whole-person care, recognized as indispensable for fostering trust within communities and effecting tangible enhancements in health outcomes. This is particularly pertinent in rural health communities, where impediments to accessing care interventions are rife, and health education exhibits significant variability.
Despite its noble aspirations, Georgia Unity encountered a slew of challenges during its inception phase. Notably, the operational limitations of community partners, including homeless shelters, emerged as a formidable hurdle. Issues ranging from deficient record-keeping practices to inconsistent utilization of technology posed formidable obstacles. Moreover, financial constraints hindered the adoption of emerging technologies by smaller community organizations, exacerbating the disparities in data gathering, sharing, and standardization—a critical concern that Georgia Unity diligently endeavored to mitigate on a statewide scale.
The inaugural year of the project yielded encouraging outcomes, indicative of its efficacy and potential for transformative impact. Notably, in Emanuel County, home to approximately 20,000 residents, Georgia Unity facilitated a remarkable 2,000 referrals to community health resources. Moreover, the initiative yielded substantial cost savings, amounting to an impressive $11,248 per person, per annum—an unequivocal testament to its efficacy and viability as a catalyst for positive change.
Moving forward, Georgia Unity remains poised to catalyze a paradigm shift in healthcare delivery, setting a precedent for comprehensive, community-driven interventions aimed at fostering holistic well-being. Through strategic partnerships and unwavering commitment, it aspires to transcend traditional healthcare boundaries, ushering in an era of inclusivity, equity, and enhanced health outcomes for all Georgians, regardless of their geographic location or socioeconomic status.
