Care Connection for Aging Services is renewing its emphasis on the Chronic Disease Self-Management Program to help older adults learn to live well with chronic health conditions.
Under a grant provided to the Missouri Association of Area Agencies on Aging, Care Connection plans to offer classes in 13 West Central Missouri counties over the next three years.
Organization information states the goal is to assemble a network of volunteer lay leaders who will continue the program into the future.
The first classes will be offered in Warrensburg.

One class will be from noon to 2:30 p.m. on Mondays, Dec. 10 through Jan. 14, at the Warrensburg Senior Center, 445 E. Gay.
Another class will be offered from 1:30 to 4 p.m. Thursdays, Dec. 6 to Jan. 10, at the Parkview Garden Apartments, 411 E. North St.
The classes are free, but registration is required by calling 1-800-748-7826.
The small-group CDSMP workshops encourage participation, mutual support and confidence building so that participants are able to manage their health and maintain active and fulfilling lives.
The course consists of six weekly sessions of two and a half hours each.
The proven practices for self-management are based on an ongoing series of studies conducted at Stanford University School of Medicine.
Topics include:
•Techniques to deal with problems such as frustration, fatigue, pain and isolation.
•Appropriate exercise for maintaining and improving strength, flexibility and endurance.
•Appropriate use of medications.
•Communicating effectively with family, friends and health professionals.
•Nutrition.
•Decision making.
•Evaluation of new treatments.
Care Connection is providing training for its lifestyles staff members and also recruiting volunteers to lead the class after training.
Each class will be taught by two lay leaders, preferably who themselves are living with a chronic health condition.
People with different chronic health problems attend the classes together.
Materials, including the book “Living a Healthy Life With Chronic Conditions” will be provided.
The book contains information and tips to help people manage chronic conditions daily while doing the things they need and want to do.
Participants make weekly action plans, share experiences and help each other solve problems they encounter in creating and carrying out their self-management program.
