Carevive’s Cancer Care Planning Tools
Carevive’s cancer care planning tools empower care teams to personalize treatment decision-making and standardize symptom assessment while on active treatment and in the survivorship period. The same is based on relevant clinical, patient-reported health data, and clinical evidence. The cloud-based technology is designed to interface with the patient’s electronic health record (EHR).
Carevive Patient Care Plans
The Carevive patient care plans and associated tools for clinicians simplify patient-centered, coordinated, and integrated multi-disciplinary cancer care. Application of such models permits outpatient cancer practices to participate in with new post-reform, value-based reimbursement programs such as the CMS Oncology Care Model (OCM).

The Collaboration Would Feature
By teaming up with Carevive, Cerner would feature embedded validated patient-reported outcomes (PRO) tools that are mapped to clinical data from PowerChart Oncology and evidenced-based content. This generates clinical decision support for the care team and personalized care plans for patients and their families.
The personalized and iterative comprehensive patient care plans from Carevive would facilitate shared treatment decision-making, and provide ongoing direction to clinicians and patients regarding symptom surveillance and management while on active treatment and during survivorship.
