Navicent Health Baldwin (NHB) has partnered with AmeriMed Mobile Integrated Healthcare to provide in-home care for patients after they’ve been discharged from the hospital.
By assisting patients after they’ve been discharged from the hospital, NHB and AmeriMed hope to improve patients’ odds of a successful recovery.
“When patients are ready to be discharged from the hospital, that does not mean the end of their care, or the end of their journey toward wellness. In fact, it is critical that their care continues once they’re discharged from an acute care hospital. Through our partnership with AmeriMed, we not only improve health outcomes but also decrease mortality,” Douglas Brewer, MD, Chief Medical Officer of NHB said.
Patients who qualify will be referred to AmeriMed for post-acute care.

An AmeriMed care coordinator will then work with the patient to schedule visits and assess their needs.
“It’s about sitting down and having a conversation with the patient. We want to learn what their needs are, we want to take the time to answer any questions they may have, and we want to make sure they have a clear understanding of their next steps on the journey to health and wellness. We’re there to assist them along on that journey,” Robert Crosby, AmeriMed Customer Success Champion said.
